- Citation (one sentence)
- Citation (approximately 350-400 words)
- Curriculum vitae of nominee
- Supporting letter from First Co-Sponsor (maximum 1 page)
- Supporting letter from Second Co-Sponsor (maximum 1 page)
The nomination process is in 3 Steps:
Step 1) complete the form below then click on " Proceed to Step 2 ";
Step 2) Review all information on the form. If adjustments need to be made, please click the "Back" button, make your adjustments and click on " Proceed to Step 2 " again. Review the information and if everything is in order, click on "Submit Nomination Now". By doing this, the nomination form is automatically sent to the GAC® Executive member responsible for that award.
Step 3) You will be asked to create a new email to be sent to GAC® Past President, Carolyn Relf - Carolyn.Relf@gov.yk.ca . This email must include ALL of the supporting documents, so please have them ready:
